Rainer Surrey 16plus Service (16plus Service) comprises two Leaving Care Teams - one based in Woking, covering the West of the county and one based in Epsom, covering the East. The Service works with 16-21 year olds in and leaving Care (and up to 24 years plus if in Higher or Further Education), who are referred by Surrey Children's Service. The 16plus Service supports Care leavers in their transition to independent living from the Looked-After system via a pathway planning model of service delivery within the context of the Children (Leaving Care) Act 2000. The Service Objectives, as outlined in its current Service Plan, are to :
The 16plus Service provides an individual Pathway Plan that addresses the full range of young people's needs to support them in preparation for adulthood and independence. Pathway planning takes into account Care leavers' needs around health, identity, citizenship, education and training, accommodation and financial needs. The service also provides a floating support scheme that aims to provide short term (not more than two years) floating support to young people to assist them to maintain their tenancies. The service is for vulnerable 16 and 17 year olds and young people leaving Local Authority Care between the ages of 16 and 21 years, who are in need of housing related support in order to maintain and promote independent living and links in with the 5 key Every Child Matters objectives:
The 16plus Service, with Surrey Children's Service, also supports an active Youth Forum. Their aim is to work with young people to ensure they receive the necessary support to reach their maximum potential. The Service seeks to develop community partnerships to enable young people to access knowledge, skills and opportunities to improve their lives so that they can participate fully in society. It listens to the views of young people and their carers to inform the Service's development and on-going review. The Rainer Surrey Drug and Alcohol Youth Support (DAYS) offers support to young people who are either at risk of exclusion or who have been excluded from school for a fixed period or permanently and where drugs and/or alcohol issues have been identified. We work with young people in the school setting, in their homes or other locations that may be needed, and provide information, education, support and guidance around drugs and alcohol and their effects. Rainer Surrey 16plus Service is managed by Rainer in partnership with Surrey Children's Service.
